Our key aim is deliver a personal, friendly and reliable alternative to the the traditional care home. That way, we help clients remain independent within the privacy of their own homes supported by highly-trained care assistants who’re on-hand to help with everyday household chores, trips to the shops, cooking, cleaning and even higher-level nursing support. We’ll even assist with social trips outside of the home.
Our elder care services are highly-personalised. Meeting with both clients and their friends and families initially, we’ll design a care plan taking every possible requirement into consideration. Once that care programme is in effect, we’ll conduct regular monitoring against mutually-agreed goals that were set out to begin with. This helps our clients maintain a level of control over the way their care is administered.
Dementia Support
London Care’s dementia teams receive specialist training to equip them with deep knowledge and understanding of the disease in its various forms and stages. This knowledge is coupled with ongoing skills training to ensure that our clients have access to the cutting-edge techniques which will help them to live safe and active lives. Our dementia support plans are always based on the needs of the individual clients, putting the person, not the disease, first.
We understand that this disease effects not only the person suffering from dementia, but also their families. We also recognise that welcoming someone into your home to care for a vulnerable loved one requires a great deal of trust. That is why we work closely with the families of all clients to develop tailored care plans and provide regular opportunities for feedback from both parties.
Extra Care
Needing access to high-quality care around the clock needn’t mean sacrificing all of your independence or privacy.
Extra Care Schemes, sometimes known as assisted or supported living, provides clients with access to high levels of care without having to sacrifice all of their autonomy. Our schemes operate in purpose-built properties across the South East which provide accessible and safe housing for older people who are unable to live completely independently. Our skilled and specially-trained care assistant teams are on site twenty-four hours a day, ready to provide support as and when it is required.
Whilst our Extra Care Schemes have a communal spirit, the care we give our clients is highly-personalised. We build tailored care plans which accommodate not just the assessed needs of the client, but also their unique preferences and aspirations. This enables us to deliver a personalised, outcome-focused service. So, whilst tenants in our Extra Care Schemes can access the benefits of their community, they can still count on getting the person-centred care they desire.
The care assistants at our Extra Care sites are passionate about creating vibrant communities in which clients can play as active a role as they choose. In addition to developing a calendar of engaging social activities, we also run scheme-wide initiatives which help service users to stay fit and healthy, such as onsite GP clinics, nutrition workshops and exercise programmes.
Physical & Cognitive Disabilities
We provide high-quality disability care for adults and children with wide-ranging issues and conditions. Offering support inside and outside the home, our highly-trained staff have the key aim of helping our clients to enjoy as active and fulfilled lives as possible. While we work to help adults in our care maintain independent, productive lives, we know too that our younger clients need to enjoy fun-filled childhoods, despite any disability.
Whether we’re caring for clients in their own homes or a residential facility, we encourage them to contribute to the choices and decisions related to their care when they can, allowing them to feel in control of their lives. We utilise a ‘person-centred’ approach when compiling our care programmes, consulting with broader family, friends and medical professionals to ensure we’re offering bespoke care to each client. In instances where clients are not older enough to make decision for themselves or do not possess the mental capacity, we will always look to make those decisions on their behalf in their own interests.
We pair each care assistant according to the client’s circumstances and requirements. In those instances where a client’s behaviour may be challenging, we’ll employ carers with the respective training while working to maintain the client’s dignity and respect. At the end of the day, it’s vital we’re pairing clients with carers who they can trust and build a relationship with.
Reablement
At some stage in our lives, many of us will require assistance in recuperating after an accident, illness, surgery or even prolonged hospital stay. At London Care, we feature specially-trained reablement teams who assist clients of all ages recuperate and regain mobility. In the long-term, our reablement programmes ultimately aim to help reduce the need for on-going care, promoting independence and self-reliance which we see as vital.
We know that coming home after a debilitating turn can mean a loss of physical strength and general confidence, even with the most ‘ordinary’ of daily tasks and activities. That’s why our reablement teams work in conjunction with a client’s doctors, physios and nursing staff to chart the best course of care to regain lost ability and confidence.
As part of the reablement process, we work with a client to set milestones to help achieve independence. Through the process, we’ll work to help a client stretch their limits week-by-week as part of agreed goals with the intent to regain the lost skills and confidence. We’ll of course amend those goals are the programmes progresses as the client’s abilities do.
